An investigation into a plane crash near the Sachigo Lake First Nation Airport has concluded.
The Transportation Safety Board of Canada has released its findings into the December 3, 2019 incident involving a North Star Air aircraft.
Investigators found that a distorted perception of risk and a results-oriented subculture among some pilots within the company contributed to the crash.
The final report also mentions risk factors related to safety management and regulatory surveillance.
Two crew members were uninjured, though the aircraft was substantially damaged.
